PARTICIPATION AGREEMENT, WAIVER OF LIABILITY, AND ASSUMPTION OF RISK Event: Renaissance Charter School at Poinciana - Bounce Hype Fundraiser Host school: Renaissance Charter School at Poinciana Date and time: Friday, April 9, 2027, starting at 8:50 AM Location: 5125 Robert McLane Blvd, Kissimmee, FL 34758 Operator: Millers Jump Time Entertainment LLC ("Operator") 1. Voluntary participation. I am the parent or legal guardian of the student(s) named in this registration, and I have full legal authority to sign this agreement on their behalf. Participation in this event is entirely voluntary. 2. Nature of the activity. I understand the event involves inflatable amusement activities, including bounce houses, inflatable slides, and inflatable obstacle courses, and that these activities are physical and recreational in nature. 3. Assumption of risk. I understand that physical activity carries inherent risks that cannot be eliminated, including but not limited to slips, falls, collisions with other participants or equipment, wet or slippery surfaces, strains, sprains, fractures, and other injury. Knowing these risks, I voluntarily assume all such risks on behalf of myself and the participating student(s). 4. Health and fitness. I confirm the student(s) are physically able to participate, and I have disclosed in this registration any allergy, medical condition, or safety accommodation that Operator staff should know about. I will not allow participation contrary to medical advice. 5. Rules and staff direction. The student(s) will follow all posted rules, the school's code of conduct, and the directions of Operator staff and school personnel. Operator may remove any participant whose conduct is unsafe, without refund. 6. Emergency medical care. If the student(s) require emergency care and I cannot be reached, I authorize Operator staff and school personnel to secure emergency medical treatment, and I accept financial responsibility for that treatment. 7. Release and waiver. To the fullest extent permitted by Florida law, I release, waive, and discharge Operator, Renaissance Charter School at Poinciana, and their respective owners, officers, employees, volunteers, and agents from any claim, demand, or cause of action for injury, illness, or property loss arising out of participation in this event, except for injury caused by their gross negligence or willful misconduct. Nothing in this agreement waives any right that cannot be waived under Florida law. 8. Indemnification. I agree to indemnify and hold harmless the parties released above from claims brought by or on behalf of the student(s) arising from participation, to the extent permitted by law. 9. Insurance. Operator maintains commercial general liability insurance for its operations. This agreement is not a substitute for the family's own health or accident coverage. 10. Governing law and severability. This agreement is governed by the laws of the State of Florida. If any provision is found unenforceable, the remaining provisions remain in full force. 11. Electronic signature. By checking the acceptance box and submitting this registration, I acknowledge that I have read and understood this agreement, that I sign it freely, and that my electronic acceptance has the same effect as a handwritten signature.
