Millers Jump Time Entertainment LLC Promotional Video Release Form

The Media Release Form is effective upon selecting "I Agree" and will remain in effect indefinitely. The ticket holder, parent, or guardian (the "Party") acknowledges and agrees to the terms below.

The purpose of this form is to request permission for our team to create promotional photographs, video, and audio for our fundraiser program, which may include your child. Millers Jump Time Entertainment LLC (the "Company") may use this content for marketing and promotional purposes.

1. I agree that photographs and video taken by the Company may be shown on the Company's website, at public, charter, or private school events, on social media platforms, and in written articles. I grant the Company the right to edit, use, and reuse this content without compensation.

2. In giving this consent, I release the Company from liability for violations of personal or proprietary rights I may have in connection with third-party use of the images or video on social media.

3. I grant the Company permission to alter the photographs, provided they are not altered in an explicit manner or used to maliciously misrepresent the Party or their associates.

4. I waive the right to inspect or approve the finished photographs, video, or related electronic materials, whether the use is presently known or unknown, and waive royalties or other compensation arising from or related to use of the image.

5. I certify that I am 21 years of age or older and that I am the parent or legal guardian authorized to make this decision for the child named in this registration.

By selecting "Yes, I grant permission," I consent to Millers Jump Time Entertainment LLC allowing my child to appear in video or audio recordings, photographs, written articles, websites, and social media.

Asking us to stop future use

A parent or guardian may ask us at any time to stop future use of a photo or video of their child. Contact us with the school, event, student name, and, if you have it, a description or link to the content. We will stop future use and remove content we control. Materials already distributed, printed, or reshared by third parties may not be fully retrievable, and we cannot promise recovery of those.

Items flagged for attorney review

• The clause stating the release "will remain in effect indefinitely"
• The clause requiring the signer to certify they are 21 years of age or older

These clauses are preserved as the owner supplied them and are flagged for attorney review rather than changed.
